EPF Form 2: EPF Nomination Form & Declaration Form
Every employee must nominate one or more members for his or her EPF account under Employee Provident Fund Scheme, 1952. Following the member's demise, the nominated beneficiary can withdraw the accumulated funds from EPF accounts. For this to happen seamlessly, the member must know how to fill out EPF Form 2 to file a nomination and declaration for the same.

Structure of EPF Form 2
There are four sections in the EPF Form 2. These include- General Information, Part – A, Part– B, and a Certificate from the Employer.
General Information of EPF Form 2
Under the general information of EPF form 2, you must provide below details:
- Member’s name
- Permanent and temporary address
- Date of birth
- Name of father or husband
- Marital status
- Gender
- Account number
- Date of joining the EPF scheme and nomination details
Structure of Part A of Form 2
In part A section of this form, an EPF account holder must provide the details regarding who will be the nominee to receive the EPF account balance after his or her death. This section includes:
- Name and address of nominee(s)
- Relationship of nominee with the member
- Date of birth
- Provident Fund amount that each nominee will receive
- If the nominee is underage, funds will be sent in the guardian’s name and address given in the Form as long as the nominee remains a minor
- Subscriber’s signature or thumb impression
Structure of Part B of Form 2
In this section of EPF form 2, you have to provide the details for nominating your family members to earn the Employee Pension Scheme (EPS) balance. You must mention the following details:
- Name and address of your family member
- Date of birth
- Relationship with the member
- Full signature or thumb impression of the member
For nomination of monthly Widow Pension (Para 16 2(a) (i) and (ii)), you have to provide the below details:
- Nominee’s name and address
- Date of birth
- Your relationship with the nominee
Certificate by the Employer
A member or an employee needs to certify the following details in this form:
- Name of member
- Authorised signature of an officer
- Designation of officer
- Date
- Place
- Name and address of the organisation and stamp
Guidelines for Nominating Family Members Using EPF Form 2
All members must follow some essential rules when they nominate family members in EPF Form 2. A married employee with his or her parents, and their dependent family members, as well as employees who don’t have parents are allowed to nominate any number of members of their family. Employees must follow these points while nominating their family members through EPF Form 2.
A male employee can fill out the nomination form for the below family members under the Employees’ Provident Fund Scheme:
- Wife
- Dependent parents
- Children
- Widow of son and children
Female employees can nominate the following members:
- Husband
- Dependent parents
- Children
- Dependent parents of husband
- Widow of son and children
In the case of an Employee Pension Scheme, a member can fill out the form for the following family members:
- Spouse of the employee
- Unmarried daughter and minor son of an employee
- Son or daughter adopted before the employee's death
How to Fill EPF Form 2 Revised for Nomination?
You must follow the below steps to fill out the nomination form online using EPF form 2:
Step 1: Go to the EPF portal and sign in with your UAN (Universal Account Number) and password.
Step 2: Click on ‘e-Nomination’, which you can find under the ‘Manage’ section.
Step 3: In the ‘Profile’ section, submit your ‘Permanent Address’ and ‘Current Address’, and click on the 'Save' option.
Step 4: Select the option asking whether you want to nominate any family member.
Step 5: Enter the details for each family member as specified on that page. Then, click on the 'Save Family Details'. You may select the 'Add row' option for adding more nominees.
Step 6: Choose which members you want to nominate from the given list. Then, enter the details regarding the EPF percentage they will receive.
Step 7: Click on the ‘Save EPF Nomination’ option to save your nominee details.
Step 8: Now, click on ‘e-sign’ to start the nomination process. Then, click on ‘e-nomination’ under the ‘Manage’ section.
Step 9: Submit your virtual Aadhaar ID and click on the ‘Verify’ option.
Step 10: Approve the request for Aadhaar’s e-KYC verification.
Step 11: Enter your Aadhaar number or virtual ID once more and click on the ‘Get OTP’ option. You will receive a one-time password (OTP) in your Aadhaar-linked mobile number.
Step 12: Enter the OTP in the required field to register nomination details with the EPFO. You do not have to submit any physical document after completing the process.
Points to Consider While Nominating Family Members
You must remember the following things while nominating family members through EPF Form 2:
- You can change the nominee names at any time.
- Subscribers can nominate any person if they have no family member.
- If the member has a family, he or she cannot nominate any individual outside his/her family.
- An employee must have a service record of at least 10 years to be eligible for a pension.
- Individuals who join an EPF-eligible organisation must join the scheme if their salary is ₹15,000 per month or more.
- If an unmarried member marries, the other member needs to file a new nomination.
- You can choose more than one family member as a nominee. The selected percentage of the EPF corpus that each member will receive must be mentioned as well.
